[Real time tridimensional echocardiography in a patient with hypertrophic cardiomyopathy].

Correspondência : Marce lo Lu iz Campos V ie i ra • Rua Cardoso de Melo , 463 /21 04548 -002 – São Pau lo , SP E-mail: [email protected] Recebido em 9/10/05 • Aceito em 17/10/05 A ecocardiografi a tridimensional em tempo real permite a visibilização estrutural cardíaca a partir da composição de múltiplos planos de observação, propiciando novas informações morfológicas e funcionais1,2. Descrevemos caso do paciente de 32 anos, sexo masculino, com diagnóstico de cardiomiopatia hipertrófi ca. A análise ecocardiográfi ca bidimensional (eco 2D) demonstrou hipertrofia em segmentos médio e basal do septo interventricular (16 mm) e a observação de eletrodo de marcapasso em câmaras direitas (fi g. 1). A medida do átrio esquerdo (plano ântero-posterior) demonstrava diâmetro de 3,2 cm (valor normal < 4 cm). A análise ecocardiográfica tridimensional em tempo real (eco 3D) confi rmou os achados ao eco 2D (fi g. 2A, 2B), e acrescentou informação com relação a anatomia atrial esquerda, demonstrando aumento atrial em seu plano de elevação (plano de profundidade), com volume atrial esquerdo de 85 ml (valor de referência em voluntários normais: 42 + 17 (18-79) ml. O eco 3D acrescentou informação estrutural neste caso de cardiomiopatia hipertrófi ca, e talvez futuramente as medidas cavitárias possam ser fornecidas em relação a volumes e não mais relacionadas a aferições em único plano de observação.
